*Please note that there is a £5 administration fee per application. This is payable either by cash (at class) or by cheque payable to Mitch Jenkins. Please enclose this amount with your completed form and return it to your venue teacher in a sealed envelope or via post to 104 Station Road, Worthing , West Sussex, BN11 1JY.  Do not send cash through the post.
